Claim for Damages

If you believe the City if responsible for damage to your person or property, you may file a Claim for Damages. This is the first step in the process to seek compensation from the City. 

HOW TO FILE A CLAIM

To file a claim, please complete the official Claim for Damages Form and submit it to the City Clerk's Office. You may download the form above, or pick up a printed copy at City Hall.

Please ensure all sections of the form are filled out completely and accurately, including:

  • Your name and contact information
  • Date, time and location of the incident
  • Detailed description of the incident and damages
  • Supporting documentation (photos, invoices, estimates, witness statements, etc.)
  • Notarized signature
    • If you need assistance in obtaining a notary, the staff at City Hall can notarize the form for you at no cost

WHAT HAPPENS NEXT

Once received, your claim will be logged, the appropriate City departments will review and document the claim and forward it to our insurance company, Washington Cities Insurance Authority (WCIA). The insurance company will subsequently contact you. After the claim has been sent to WCIA, all inquiries regarding the status of the claim must be directed to them at (206) 575-6046.

Please note: Filing a claim does not guarantee payment. Each claim is evaluated based o the facts, applicable laws, and any supporting documentation provided.
